CSS Health (a division of Clarest Health) is a software company that supports health insurance companies by providing software solutions and outsourced services. We license software to help manage medical programs, particularly the Medication Therapy Management (MTM) Program required by CMS for Medicare Part D. We also offer outsourced MTM services, utilizing our team of pharmacists, clinicians, and support staff to conduct Comprehensive Medication Reviews (CMRs) for our clients' members. These reviews are crucial for maintaining high star ratings in Medicare Part D programs.
The Clinical Interviewer - Pharmacist reports directly to the Supervising Pharmacist and provides individualized MTM services including, but not limited to: Comprehensive Medication Reviews, Adherence/Targeted Interventions, and Transitions of Care. The MTM Pharmacist will also assist with finalizing reviews completed by clinical staff members and assisting with Comprehensive Medication Review quality assurance.
Location: Remote eligible in AR, CA, FL, GA, IL, ME, MI, MO, NY, NC, OH, OK, OR, PA, TX, WV and WY
Salary: $40.80 per hour
Schedule: Mon-Thurs: 11AM - 7PM | Fri: 9AM - 5PM Or Mon – Fri (9AM – 5PM)

Key Responsibilities:
- Delivering MTM services including but not limited to: Comprehensive Medication Reviews and Adherence/Targeted Interventions, and Transitions of Care to assess medication appropriateness and optimize medication therapy
- Performing accurate medication reconciliation/allergy review
- Provide patient education/counseling on chronic disease states (diabetes, hypertension, dyslipidemia, COPD)
- Assisting patients with medication costs using interventions such as formulary and therapeutic interchanges, pill splitting opportunities, and evaluating for unnecessary and/or duplicate medication therapies
- Assessing prescribed medication therapy for unnecessary medications, therapy gaps, ineffective/harmful medications, inappropriate doses, adverse drug reactions, and adherence
- Collaborate with the interdisciplinary care team, patients, and/or caregivers to identify and develop patient-specific disease state therapy goals
- Determine appropriate interventions to resolve medication therapy problems, achieve goals of therapy and prevent new medication therapy problems
- Finalizing comprehensive medication reviews completed by clinical staff members (LPNs, RNs, pharmacy student interns)
- Overseeing call center operations and communicating with CSS staff members to ensure that standard operating procedures are met
- Performing quality assurance reviews for clinical staff members, providing continuing medication education, and participation in clinical staff meetings
- Efficiently and accurately documenting all services provided
- Other duties may be assigned with company growth
- The ability to adhere to Clarest’s Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer
Qualifications:
- BS in Pharmacy or PharmD from an accredited school of pharmacy
- Current license to practice Pharmacy in the United States.
- Fluent (speaking at health care professional level and writing) in English.
- MTM experience preferred but not required
Skills + Abilities:
- Knowledge and understanding of the 5 Core Elements of MTM services – 1) Medication Therapy Review – Comprehensive and Targeted, 2) Personal Medication List (PML), 3) Medication-Related Action Plan (MAP), 4) Interventions, and 5) Documentation of Services
- Strong clinical therapeutic skills and knowledge of appropriate medication dosing in geriatric and adult populations
- Must be able to work independently and in a collaborative environment
- Strong Telephonic/Listening/Conflict Resolution Skills – Must be able to speak clearly and communicate both verbally and in writing with senior citizens, patients, and staff. Must be empathetic and exhibit active listening techniques, and be able to resolve potential patient conflict utilizing an appropriate tone
- Professional demeanor – Must be able to consistently maintain professional behavior while being dependable/reliable
- Self-management Skills – Self-motivated, organized, detail oriented, and manages time and resources effectively
- Knowledge of the Law/Strong Work Values – Must abide by state pharmacy law and CMS compliance measures, and ensure privacy and confidentiality as required by HIPAA and company guidelines. Must be able to follow all company and departmental processes and procedures
- Computer Competency – Must be able to efficiently multi-task between several different computer applications
