We're seeking a dynamic per diem Pharmacist to join our team and make a difference in the lives of residents in long-term care facilities.
Location: Melville
Salary range: Starting at $60/hour
Coverage needs:
- 3:00 pm – 12:00 am
- 5:30 pm – 2:00 am
- Weekends 8:00 am - 8:00pm

Key Responsibilities:
- Ensure PV2 meets pharmacy run-time commitments and exceeds expectations.
- Conduct final prescription reviews before toting.
- Verify E-Kit contents and pull control medications as needed.
- Process and compound sterile/non-sterile products.
- Oversee new and refill IV orders and supplies.
- Track IV pump returns and process backorders.
- Perform final reviews for OOP meds, REMs submissions, and Clozapine orders.
- Check pre-packaged medications and oversee floor technicians.
- Assist pharmacy technicians and be on-call as scheduled.
- Ensure compliance with federal/state pharmacy laws and policies.
- Report medication errors and participate in quality improvement.
- Maintain inventory accuracy, IV room standards, and narcotic room cleanliness.
- Review and order IV supplies and C2 products; report shortages.
- Process and sign in controlled medications, including destruction and distribution.
- Flex into PV1 role as needed.
- Complete QA sheets and perform other assigned duties.
- Must adhere to Clarest’s Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer
Qualifications:
- Pharmacist license to practice in state required (must be in good standing)
- Experience in long term care is preferred
- Experience with Frameworks and Docutrack is preferred

- Must have the ability to employ critical thinking skills and ability
- This position includes access to highly confidential information including protected health information (PHI) - the Associate must follow Federal and State HIPAA/Privacy laws and maintain compliance with the company’s policies and professional ethics
- Ability to exert up to 40 lbs. of force occasionally and/or lift, carry, push or move objects and stand or perform data entry on a computer for extended periods of time during shift
- This position requires the ability to understand direction and adhere to established procedures
- This position requires near vision acuity and color recognition
